This program is ideal for adults who need ongoing primary care for common medical conditions such as high blood pressure, diabetes, high cholesterol, thyroid disorders, asthma, anxiety, depression, and other routine health concerns while waiting to establish with a permanent primary care provider.
Before your first appointment, we'll review your completed health history, current medications, and intake forms to ensure this membership is the right fit for your healthcare needs and to make the most of your initial visit.

Program Guidelines

Designed to continue and manage existing medical conditions, not to perform comprehensive evaluations for new or complex diagnoses.

1-2 problems can be addressed at a visit. We will reschedule you for additional needs.

No prescribing or long-term management of controlled substances.

Does not include hormone replacement therapy (HRT), medical weight management, aesthetic services, or member discounts associated with those programs. These services are available separately.

Patients requiring complex diagnostic workups, ongoing specialist-level care, or management beyond the scope of this program may be referred to the appropriate healthcare provider.
